The Hospice Team

Who makes up your local hospice team?

  • The patient and family.
     
  • Patient Physician—participates in the plan of care and remains the patient's primary physician.
      
  • Medical Director—oversees hospice care, advises the hospice team, and is available to the primary physician for consultation.
     
  • Registered Nurse—assesses and manages pain and other symptoms related to the disease under the direction of the primary physician. Makes regular visits, teaching and educating caregivers on how to care for their loved ones.
     
  • Home Health Aide—provides personal hands-on care and assistance with activities of daily living, which may include feeding and bathing.
     
  • Social Worker—addresses the psycho-social needs of patient/family facing terminal illness. Assists with legal, financial and social resources.
     
  • Spiritual Support—provides support and comfort directly or contacts local clergy at request of patient or family.
     
  • Bereavement Support—trained to provide support and companionship. Assists with day-to-day activities.  
     
  • Volunteer—provides companionshp to patient and family.
